1. Fixed a problem that sometimes the big cursor is not enabled when selected.

2. Enabled cursor animation for big cursor. This causes a limitation that hourglass cursor will not be changed when you change between big and normal cursor. The hourglass cursor used when Windows is started will be used.

3. Added support of 85Hz refresh rate.

4. Added support to only enable modes returned by BIOS function 5f16h. If this BIOS function is not supported, enables modes based on controller and DRAM size.

1. Modified so that only the refresh rate for current mode is set when CHIPSCPL is started, because 65548 BIOS uses same bits to store refresh rate information of mode 40 and 41, and etc. Previous CHIPSCPL set refresh rates for modes which are listed in CHIPSCPL.INI file. This causes a problem when you set different refresh rates for mode 40 and 41.
